Fond du Lac Middle School Co-Curricular Code

All parents and students must read the Fond du Lac School District Co-Curricular Code (or read the presentation linked within the athletics section), sign and return appendix A, B, C, and D of this link before participating in co-curricular activities at Theisen Middle School.

Middle School Physical Form

Each student is encouraged to have a physical form filled out by a doctor and on file at Theisen Middle School in order to participate in any activity, club, or sport.  Physicals must be after April 1, of the current year. There only needs to be ONE physical documented for all three years of middle school.
